Several of my books are available both from here and Lulu.com  in a variety of formats as shown below.

For larger books the coil bound version is without doubt the best for lying flat on the keyboard rest.

Note that the coil bound versions are not available from Amazon, Wordery, Walmart or W.H. Smiths etc.
